Patient had a previous breast augmentation in 2019. After losing weight she noticed some changes in her breast that she didn’t like. This included some thinning of her tissues, which made the implant more visible. We also call this rippling. The arrow underneath the right breasts in the before picture shows where she was really noticing this. Easier to notice on physical exam than it is sometimes in a photograph. She also wanted to decrease the space between her breasts shown with the central arrow.

Her initial surgery was done with the standard silicone gel implant. We elected to treat this for her with three different techniques: Implant exchange, pocket revision, fat grafting.

Implant exchange to a more cohesive silicone device(aka gummy bear implant). In my experience, this can often help with decreasing the appearance of some soft tissue rippling. She wanted a little more cleavage so we also tightened up the lateral portions of her implant pocket. Lastly we also used fat grafting to increase the thickness of her tissues inferiorly and medially.

Photographs are four months after surge. By combining all three techniques, I think we achieved a really nice improvement for her!
